Services:

Registering with the Practice:

Simply sign and bring your medical card to the surgery. Initial registration takes place between 2.00pm and 5.30pm Monday to Friday.

On registering, you may be asked to complete a questionnaire because your notes can take some time to catch up with you. An appointment with the practice nurse for a health check will be made and, at the same time, you will be given an opportunity to ask any questions.

Change of Personal Details:

If you change your name, address or telephone number, please inform us using one of the following methods:

If you move outside of the practice area it may be necessary for you to find a doctor in your new area.
Failure to advise us of a change of details may result in your name being inadvertently taken off the doctor’s list.

Private Medical Services:

Some services we provide do not come within the treatment available under the NHS. These services are subject to a charge dependent on the time required. Examples include:

1	Private medical examinations.
2	Health Reports.
3	Doctor’s certificates for less than 7 days.
Please Note: As a matter of policy, the practice does not provide signatures on shotgun certificate and passport applications.
