Please note that a survey of patients will be conducted in September/October2008. The survey will focus on two main areas of care:

• The patient’s experience of the practice – waiting times, ease of getting appointments, continuity of care etc;
• The patient’s experience of the doctor, in particular communication skills, and inter-personal aspects of care.

The practice will be assisted in conducting the survey by ‘The Friends of the Hamilton Practice’.

Patients will be asked to fill out the form following consultation with the doctor/nurse. This should only take a few minutes.

Patient confidentiality is paramount and all responses will be dealt in strict confidence. Patients should not write their name on the questionnaire.

Pens and clipboards will be provided and completed questionnaires may be returned to a collection box.

Each form will be numbered individually purely for administrative purposes.

The results of the survey will enable the practice to identify potential improvements in the current level of care.
